HANCOCK, Mich. (WLUC) – There are still a few days left to submit the Upper Peninsula Health Surveys. Health surveys were mailed out at random to all 15 U.P. counties earlier this month.
Out of the 24,000 surveys mailed out, 4,500 have been returned so far.
The Upper Peninsula Health Department is hoping to receive at least 300 surveys for each county. The surveys will help the health department design programs to address community health needs.
“The surveys are completely anonymous. We’re not interested in an individual’s health, but in population health,” said Survey Coordinator Ray Sharp. “Did you go to the doctor in the last year? Have you visited a dentist? And then, also, how do you see your community? What are the most important needs that you can identify for your neighbors and your town?”
The surveys can be submitted online or by mail. Those who submit their survey with the provided code will be entered to win a $50 grocery card.
The deadline for submissions is September 8th.
